Persons Who May Submit the Notification

Applicant, member of the applicant’s household, a proxy (※requires a Power of Attorney).

Necessary Documents

  1. Residence Card(s) (Please bring a passport if you have not yet been issued a Residence Card)※For all family members
  2. Personal ID if submitting a notification as a proxy.
  3. Personal Seal(only applicable for those who wish to register a Personal Seal).

Changing Residence within Japan

Persons Who May Submit the Notification

Applicant, member of the individual’s household, a proxy(※requires a Power of Attorney).

Necessary Documents

  1. Proof of Change of Residence(issued at your previous residence)or a Basic Resident Registration Card (Juki Card) (must have been updated upon submitting a Moving Out Notification at your previous place of residence)
  2. Residence Card(s)(for all family members)
  3. Personal ID if submitting a notification as a proxy
  4. Personal Seal(only applicable for those who wish to register a Personal Seal)

Moving Within Nagasaki City

Persons Who May Submit the Notification

Applicant, member of the applicant’s household, a proxy(※requires a Power of Attorney).

Necessary Documents

  1. Residence Card(s)(for all family members)
  2. Personal ID if submitting a notification as a proxy
  3. National Health Insurance Card(only applicable to those who possess one)
  4. The My Number Card or Basic Resident Registration Card, "Juki Card"(only applicable to those who possess one)

Moving out of Nagasaki City

Persons Who May Submit the Notification

Applicant, member of the applicant’s household, a proxy(※requires a Power of Attorney).

Necessary Documents

  1. Personal ID of individual submitting notification
  2. National Health Insurance Card(only applicable to those who possess one)
  3. The My Number Card or Basic Resident Registration Card, "Juki Card" (only applicable to those who possess one)

Regional Offices, Kurosaki Office, Ikeshima Office, Nagaura Office, and Area Offices.

Turning On Water and Sewage (Moving In)/
Turning Off Water and Sewage(Moving Out)

A notification must be submitted when you wish to turn on water and sewage, and when you wish to turn off water and sewage.

Applications may be made any time via the internet.
However, applications will be processed during office hours.

Enrolling in the National Health Insurance(Moving In)/
Withdrawing from the National Health Insurance(Moving Out)

National Health Insurance is for those who are not enrolled in workplace insurance etc.
Those enrolled in the National Health Insurance pay a portion of any medical fees incurred through sickness or injury. The remaining costs are covered by the National Health Insurance.

Necessary Documents

  1. Passport or Residence Card ※necessary for all new arrivals to Japan
  2. Proof of previous year's income(a withholding slip)※not necessary for new-arrivals to Japan
  3. Personal Seal ※signature is acceptable
  4. Moving In Notification
  5. National Health Insurance Card(s) for anyone in your household already registered for the National Health Insurance
  6. Pension Certificate, if you are receiving your pension

Withdrawing from the National Health Insurance

Necessary Documents

  1. National Health Insurance Card
  2. Personal Seal ※signature is acceptable
  3. Moving Out Notification

Enrolling in the National Pension Fund(New Residents from Abroad)/
Withdrawing from the National Pension Fund(Residents Leaving to go Abroad)/ Change of Domestic Address

The National Pension Fund provides lifestyle and injury security for aged residents.
All residents of Japan between the ages of 20 and 60 must enroll in the National Pension Fund.

Applications for Children's Allowance(Moving In)/
Recipients of Children's Allowance(Moving Out)

Children's Allowance is given to those who are raising children until they graduate from Junior High School (up to the 31stMarch in their 15thbirthday).

Applications for Children’s Allowance

Application can be made at the Child Policy Division Nagasaki City Hall or at Regional Offices.
Please make applications for Children’s Allowance within 15 days of moving into Nagasaki City.

Recipients of Children’s Allowance

Please complete procedures with the Child Policy Division Nagasaki City Hall or at a Regional Office.

Those who are Pregnant, or with Children Under 1(New Residents)

We shall issue Maternal and Child Health Handbooks to those who are pregnant or who have children under 1 years of age.

We have regular consultation days when we provide an English-language interpreter for pregnant women and expectant women and mothers, who cannot read or write Japanese.

  • Date and Time: 13:00-15:00, 3rdWednesday of every month
  • Venue: Infant Checkup Room , 2nd Floor City Hall
  • Details: Consultations regarding pregnancy, parenting, health checkups, vaccinations etc.
